BACKGROUND
A.R.S. § 49-1054 (C)
Payments to an owner or operator shall be based on the schedule of corrective action costs in effect on
the date a contract for corrective action work was entered into by the owner or operator, or if that date
cannot be determined, the date work associated with the corrective action was performed.
CONTENT
The one page Contract Form to include in your SAF application is located on the SAF web site, under
the heading State Assurance Fund Forms.
