Ward , M.D. , M.P.H . stating that: 1) a certificate of record of death shall be a true transcript of the entire record , including a copy of both the original certificate and copies of all amendments ; 2) a certificate of the record of birth shall not be a bopy but rather a certificate as to what the record shows the required items to be at the time such certificate of record is supplied to any applicant . No statement as to any amendments need be added to a certificate of the record of birth or death . Also, a recorded certificate of birth or death can be changed or altered by a statement of fact which is properly dated , signed by the person making such statement and witnessed , and which is placed upon the records of the State Registrar .
