Note: Information in this section
pertaining lo PhoneCenter Stores, deposit
and credit policies, types of local exchange
services, and service connection charges
applies to Residence service. For
information about business service, please
call your Business Service Representative
or your Business Service Center.
Visit your PhoneCenter Store
You can see all of our telephones at any of
the PhoneCenter Stores listed previously. If
you have modular jack outlets, you can take
your telephone sets home and install them
yourself.
Call your Business Office
If you prefer to call us, Service
Representatives in your Business Office can
help with orders for service and equipment,
billing matters, rates, and inquiries.
Residence customers should call the
appropriate number from the list of Business
Office numbers. Our representatives are
(here to see that your needs are met.
Credit information is important
In addition to your name and address,
Mountain Bell will need to know if and when
you have had telephone service. You will
also be asked for a number where you can
be reached, such as your telephone number
at work or the number of a close friend or
relative who could get a message to you.
If you have not had telephone service
previously, or if Mountain Bell cannot verify
your service, a series of credit questions will
be asked, such as: "Where are you
employed?'' "What credit cards do you
have?'' etc.
A deposit may be required
A deposit will be required if you have had
an unsatisfactory payment record with any
telephone company, or if you have not
supplied adequate credit information.
The deposit amount is based on the
average billing for two month's local service
and two month's long distance charges for
customers in your state.
If your service is disconnected due to late
payment, and there is either no deposit or
an insufficient deposit, a new or additional
deposit may be required to continue service.
You also have the option of providing a
letter from a person willing to guarantee
payment of your bills if that person has
established credit.
Deposits earn 6% interest
A deposit with six percent simple interest
per year, is refunded within one year if your
phone bills are paid by the date listed on
the bill. The deposit is held as security if
credit has not yet been established. If
telephone service is disconnected, the
deposit with interest is applied to your final
bill.
Prompt payment means good credit
Your credit standing is determined by your
promptness in paying your telephone bill.
Payments should be received at the
telephone company by the payment date on
your bill. No telephone credit information is
given out to other businesses.
Types of home telephone
service
The following are basic phone service
available in the Flagstaff area:
One party - unmeasured (ffat rate)
This single party service allows you to make
all of your local calls for the same charge
each month.
Two party - unmeasured (flat rate)
This party line service also allows you to
make all of your local calls for the same
charge each month. You share the line with
one other customer.
Four party - unmeasured (flat)
Eight party • unmeasured (flat)
These party line services also allow you to
make all of your local calls for the same
charge each month. You share the line with
as many as three other customers (four
party) or seven other customers (eight
party). These services are only available in
certain outlying areas.
In addition to the flat monthly charges,
mileage charges may also be applicable for
customers living outside local exchange
boundaries. Consult your service
representative for details.
Please check with your business office or
PhoneCenter Store for availability of
services in your exchange.
Charges to install and
change your service
The one-time charges to install or change
your service may vary, depending on the
type of work that is done. If you prefer, you
may be billed in four equal installments of
not less than $10 per month.

These drawings show you how to install or
disconnect your telephones.
Drawings one through four show standard
wall sockets, or jacks, for modular telephone
cords. To install your phone, match the plug
at the end of the cord with the jack, and
insert. To disconnect your phone, squeeze
the clip on the plug and pull it out of the
jack.
Drawings five and six show different kinds
of jacks, called non-standard jacks. You can
use these with modular phones by putting
an adapter in between. Plug the adapter
into the jack, then plug the cord into the
adapter.
A jack for a wall mounted telephone is
shown below. To connect your wall phone,
match the holes in the back of the phone
with the rivets on the wall mounting. Push
the phone lightly against the mounting and
slide it down into place. To disconnect, slide
the phone up to lift it off the rivets, then pull
it away from the wall.

We'll pre-wire a new home or
addition
If you are building a new home or
remodeling your home, we can install
telephone wires before the wall is put up.
Often this means your telephones can be
installed more neatly. Call your Service
Representative for this service.
No charge to disconnect service
There is no charge to disconnect service.
Call your Service Representative to
disconnect your phone. If you wish, charges
can be stopped the day you call. Ask your
Service Representative about the Set
Recovery Plan. You may be eligible for a
credit under this plan.
Charges for repair service
Repair service is available at no charge for
telephones you lease from us or for the
working parts of Design Line Telephones.
After six months, the warranty on the
Design Line housing expires and there's a
charge for repairs to it. Mountain Bell does
not repair phones bought from any retail
outlet not connected with the Bell System.
If Bell System phones in your house have
plug-in jacks, you can unplug one that
doesn't work and take it to the PhoneCenter
Store, where it will be replaced. If you
deliberately or negligently damage your
telephone, there will be a charge to fix or
replace it.

names listed on one line of the directory at
no additional charge.
Custom Calling Services
Check with your Business Office to see if
Custom Calling is available in your area.
1. Call Waiting
When you are talking on the phone, a
special tone tells you that another party is
calling. By depressing the switchhook, you
can "hold" the first call while answering the
second. This provides the benefit of a
second line at less cost.
2. Calf Forwarding
You can transfer your incoming calls to any
telephone number.
3. Three-Way Calling
You can add a third party to your
conversation. This service can save the
expense of some conference calls.
4. Speed Calling
You can program your phone to dial
frequently called numbers using only a one-or
two-digit code.

Special services for disabled people
If you are disabled, or if you know someone
who has a physical problem, we want to
help. The solution may be provided by
special equipment we already have
available. If not, we will do our best to
design an efficient solution.
Two service centers for Mountain Bell
customers with special needs are now open.
The Special Needs Center for Flagstaff is
located in Phoenix at 211 North First
Avenue.
The service center is staffed by Mountain
Bell employees specifically trained to
accommodate disabled individuals. All Bell
System products designed especially for
use by the disabled, as well as products
designed for general application that can be
used to assist disabled customers, will be
available through the center.
(Customers with special needs have the
option of either visiting the center in person
or calling toll-free.) Facilities for both voice
and teleprinter communication with our
Service Representatives are available. To

In addition, after June 29, 1980, Mountain
Bell will provide Operator assistance to its
customers who use telecommunications
devices for the deaf (TDD/TTY). Any

directory assistance or other Operator-assisted

This number may also be used for after-hours
Business Office and repair service.
Consumer rights
How to complain
Your Service Representative has a basic
responsibility to answer your questions and
resolve your problems. If you're not
satisfied, feel free to ask for a Supervisor. If
your problem can't be solved by the
Supervisor, ask for the Manager.
If you are still not satisfied, you have the
right to file a formal or informal complaint
with the State Corporate Commission of
Arizona, which regulates Mountain Bell. The
Corporation Commission can be reached at

Credit for loss of service
If you have reported that you are unable to
make or receive calls and your phone is out
of service twenty-four hours or more you are
entitled to a credit on your bill for the local
service charges.

Consumer responsibilities
Providing your own equipment
Under the Federal Communications
Commission's registration program, you may
supply your own telephone or other terminal
equipment and connect it directly to the
nationwide telephone network.
The equipment must be approved by the
FCC and you are responsible for its proper
connection, maintenance and repair. FCC
rules also require that you notify the
telephone company before connecting your
equipment.
Private equipment may not be connected to
coin phone service and party lines.
Yield a party line in an emergency
State law requires you to yield a party line
immediately when told the lines is needed
for an emergency. It is also unlawful to take
over a party line by stating falsely that the
line is needed for an emergency. The law
defines an "emergency" as "a situation in
which property or human life is in jeopardy
and the prompt summoning of aid is
essential." Anyone violating this law in
Arizona is guilty of a class 2 misdemeanor
and is subject to a fine of not more than
$750 or confinement in the county jail for
not more than four months, or both.
For your protection
Our employees carry company
identification cards.
For your safety every Mountain Bel!
employee is required to carry an
identification card showing his or her name,
and a photograph. The people who deliver
phone books do not work directly for
Mountain Bell, but they do carry
identification. If you have any doubt whether
people are from the phone company, please
ask to see their cards.
How to handle obscene or
harassing calls
If you receive obscene, harassing or
threatening calls, follow these suggestions:
1. Hang up at the first obscene word or if
the caller doesn't say anything the second
time you say hello.
2. Give no information such as your name
and address until the caller has been
identified.
3. Advise your children to give no
information to strangers, if you're not home,
they should say, "Mother/Father can't come
to the phone right now."
4. If these annoying calls persist, call
your Business Office or your Police
Department, but if you receive calls
threatening injury to a person or damage to
property, call the Police immediately.
It is a crime under both Arizona and federal
laws for anyone to make obscene or
harassing phone calls. These laws impose
penalties of up to six months imprisonment
and /or up to a $1,000 fine.
How to handle unwanted sales calls
If you do not want to talk with a person
selling a product or service by telephone,
just say "No thank you," and hang up.
Fraudulent callers are subject to
prosecution
It is illegal for another person to charge long
distance calls to your number or credit card
without your permission. Persons placing
such calls are subject to prosecution, and if
convicted are subject to imprisonment of up
to six months and a fine of up to $1,000.
Recorded calls require a beep tone
If your conversation is being recorded for
business or other reasons you will hear a
beep tone every 15 seconds. Use of a
recorder without a recorder-connector
containing a beep tone warning device is
not permilted, except on emergency
reporting systems, including Police and Fire
department lines.
Unlawful wiretapping is subject to
prosecution
It is a crime under federal law for any
person, including a telephone subscriber, to
wiretap or otherwise intercept a telephone
call, unless that person has first obtained
the consent of one of the parties actually
participating in the call. Properly authorized
law enforcement officers can engage in
interceptions without the consent of either
party when proceeding under court orders
issued pursuant to applicable provisions of
federal law. Under federal law, the penalty
for illegal wiretapping can be imprisonment
up to five years, a $10,000 fine, or both.
Mountain Bell does not install wiretaps for
anyone, but when a court orders a wiretap,
we are required by law to cooperate.
We believe in your right to privacy and will
do everything v/e can to protect it.

will sound 10 seconds later.
To answer the waiting call:
You may terminate the existing call and
hang up. Your phone will ring. Answer it and

hold an existing call, depress the
switchhook firmly and release it
immediately.

To remove either call:
After you've finished talking with either
party, hang up. Your phone will ring, and
when you answer, you'll be connected with
the remaining person.

1) LISTEN FOR DIAL TONE
2) PUSH 74 and BUTTON (Touch-
Tone telephones)
DIAL 74 (Rotary Dial telephones)
Listen for a second dial tone.
3) DIAL THE SPEED CALLING NUMBER,
plus all digits of the telephone number
(including the access and area code if
long distance). Two short bursts of tone
followed by dial tone confirms that the
code and telephone number are
recorded.
NOTE: The telephone number you record
on your Speed Calling list must have at
least seven (7) digits. For example,
Directory Assistance or 911 emergency
service cannot be recorded.

How am I billed?
Charges for your local service and
equipment are billed one month in advance.
Message units, long distance and telegrams
are billed from the last bill date through the
day prior to the current bill date: charges for
installing or changing equipment are billed
the same way.
What is the Due-by Date?
This is the date by which Mountain Bell
should have your payment, not the date you
should mail it.
What happens if I can't pay my
bill on time?
If you can't pay by the Due-by-Date call
your Service Representative to discuss
payment arrangements.
Mountain. Bell does not apply an extra
charge for bills paid after the due date, but
repeated late payments may damage your
credit record with us. You may be requested
to pay a ueposit or art additional deposit as
a result of late payments. Late payments
may also result in your phone service being
disconnected.
How can I get service restored iff it
has been disconnected for
nonpayment?
All charges billed for service must be paid
before your service can be reconnected.
This could include any amount which you
may have received on a new bill. You will
also have to re-establish your credit and as
a result, we may require a deposit before

Charges for Directory
Assistance
Most Arizona customers have a basic 5 call
allowance each month for calls to local
Directory Assistance (1+411) and for
numbers within the 602 area (1 + 555-
1212). Directory Assistance calls over the
allowance cost 10 cents each. Directory
Assistance placed through the "0" Operator
cost 20 cents each. Charges will apply on
requests for numbers which are unlisted or
which the operator cannot find.
Directory Assistance charges do not apply
to calls from hotels, motels, hospitals,
nursing homes, and coin telephones.
There is no charge for Directory Assistance
calls outside Area Code 602.
The blind and physically disabled may apply
for exemption from these charges through
the Business Office.
Business customers with PBX service have
an allowance of 5 calls per trunk.
Five ways to save on
Directory Assistance
charges
1. Write down the numbers that you call
frequently. You may wish to circle or
underline a number in your directory to
quickly find the number a second time.
2. To get the telephone number of
someone who has recently moved, call the
old number. A new number reference is
generally provided.
3. If you need two numbers, request them
both at the same time. Once a call to
Directory Assistance has been placed, a
second listing is given at no charge.
4. You can save the cost of a second call
to Directory Assistance if you can supply
complete information the first time: exact
location, accurate spelling and full names.
5. If you are not sure of the name or
spelling of a restaurant, theater, store,
union, or business, check the Yellow Pages
under the appropriate heading.

Credit for poor connections or

When you dial long distance and reach a
wrong number, have a poor connection or
other problems, hang up and dial "0" (Operator)

Types of Operator-assisted
calls
Collect calls
You may call collect if the person or firm
you are calling agrees to pay the charges.
Dial the call and when the Operator comes
on the line, say you're placing a collect call
and give your name. The Operator will leave
the line when the person you've called accepts
the charges.
Credit card calls
If you have a telephone credit card, you can
have long distance calls charged to your
regular monthly telephone bill. Dial the call
and, when the Operator comes on the line,
say you are placing a credit card call and
give your credit card number. Credit cards
may be obtained from your Business Office
at no charge.
Third-number calls
You may have long distance charges billed
to an authorized third telephone number.
Dial the call and when the Operator comes
on the line say you'd like to charge your call
to a third number and give the Operator that
number including the Area Code.
Collect, credit card, and third number
calls can be placed either station-to-station
or person-to-person.
Time and charges
When placing a long distance call you may
ask the Operator to tell you Ihe length of
lime you talked and how much it cost. Dial
the call and when the Operator comes on
the line say that you will want time and
charges when the call is completed.
Person-to-person calls
This is the most expensive type of long distance
call. Call person-to-person when you
wish to reach a particular person or extension
number. Dial the call and, when the
Operator comes on the line, say you are
making a person-to-person call and give the
name of the person you want to talk to. The
Operator will leave the line when Ihe person
you're calling has answered Ihe telephone.
Mobile and marine calls
You can make local and long distance calls
to automobiles, trucks, aircraft, boats and
ships which have phones. Some calls can
be completed by dialing direct; others require
calling the Operator. For information
on other communication companies look in
the Yellow Pages, under the headings of
"Paging and Signaling" and "Radio-telephone
Communications."
Conference calls
You can talk with several people in different
places at the same time by dialing the Operator
and saying you wish to make a conference
call. Give the Operator the names and
telephone numbers of the people you want
to talk to. You may want to arrange a calling
time in advance.
International calls
You can call practically anywhere in the
world by dialing the Operator.

New Year's Day/July 4th/Labor Day/Thanksgiving/Christmas Day.
On these legal holidays, the evening rate applies all day on dial-direct
calls unless a lower rate would normally apply (nights &
weekends). On Operator-assisted calls, the dial-direct discount rate
applies only on additional minutes.
To place a call to the Bahamas, Bermuda, Canada,
parts of Mexico (903 and 905 area codes only), Puerto
Rico and the Virgin Islands
Calls to these points can be dialed in the same manner as long
distance calls within the United States. Simply dial "1" plus the
appropriate Area Code and the local number.
Highest rates —
Operator-assisted three-minute rates
Operator-assisted calls are those requiring the assistance of an
Operator to complete the call. These include person-to-person, coin,
collect, credit card, billed to a third number, hotel guest, and time
and charge calls.
The initial period for all Operator-assisted calls is three minutes
and the full (weekday) rates apply all days at all hours.
Additional minutes are charged at dial-direct rates, and include the
same discount applicable to the time period when you call.

You can spread your payments for
installation charges and equipment equally
over a four month period. The minimum
monthly payment is $10.
Install everything at once
Every time you have telephones installed or
service changed, certain one-time charges
apply. Save money by having installation
work done at one lime.
Buried Cable
Many of our phone lines are buried. If you're
going to do any digging, please call for
assistance in locating our cables before you
dig. The number is on page 3 of the "Doing
Business With Us" section. If you cut a
cable, you will be billed for repair costs.
Save money on basic service
Choose a telephone service that is best for
you. (See page 4.) If you are a light user of
the telephone and don't mind an occasional
inconvenience, ordering party-line service
can save you money.

It'll save you lots of time when you want to
call back to friends or business associates
you left behind —you'll already have their
numbers and wont have to call Directory

Make special arrangements when
you go on vacation
If you are going to be gone for an extended
period of time, call your Service
Representative to see if you should have
your phone put on vacation rate service.
The vacation rate is lower than most regular
rates and may save you money. Your calls
may be referred to another number while
you are away.
Call during bargain times
Rates on direct dial long distance calls are
lower during evening, night and weekend
hours. Also, take advantage of holiday rates
on New Year's Day, July 4th, Labor
Day.Thanksgiving and Christmas Day. See
the rates on pages 17 and 18.
Schedule and plan your calls
If you call someone frequently, try to set a
specific time when he or she will be there to
receive your long distance call. That way,
you can always call station-to-station and
save. Plan the things you want to say to
save calling time.
Check your directory
Before making a long distance call to a
business in another town, check your phone
book to see if the business has a toll-free
number. If they do you'll save yourself the
price of a long distance call.
Dial direct and save
If costs less when you dial long distance
calls yourself.
Collect, credit card and coin phone calls
require the services of an Operator: so do
requests for time and charges and calls
billed to another number. Calling person-to-person
always costs the most.
Ask for credit for wrong numbers
When you dial long distance and reach a
wrong number, hang up and dial "0"
(Operator) right away. Explain what
happened so the Operator can arrange for
the proper credit. The faster the problem is
reported, the easier it will be to give you
credit.
If you are billed for a long distance call that
you did not make, call your Service
Representative.
Ask for credit on poor connection
If you have a poor connection or are cut off
on a long distance call, both parties should
hang up. If you made the call, tell the
Operator and ask to be reconnected. You
will also get an adjustment.
Ask for coin phone refund
If one of our coin phones collects your
money but you don't talk to anyone, report it
by dialing the "0" (Operator) from another
phone. We'll see that the phone gets fixed
and mail you a refund.
Protect your credit card
Take care not to lose your telephone credit
card or divulge your billing number.
Although it is against the law to defraud the
telephone company, some criminals still try
it. Be certain to check your long distance bill
and alert us promptly if there are any
unlikely charges.
Save time and trouble
With the Bank Draft Payment Plan you can
have your bank automatically pay your
monthly telephone bill and deduct the
amount from your checking account. You
get a statement from us marked "Paid'' and
a proof of payment from your bank. Call
your Service Representative for more
information.
Look in the book
You can avoid Directory Assistance charges
by looking up your own numbers in the
directory. Also, jot down frequently called
numbers and keep them next to your
telephone.
Hang up carefully
Be certain to replace the handset properly
on the telephone after each call is
completed. If your phone is slightly ajar and
off the hook, it will stop calls from reaching
you.
Teach children emergency tips
If you have children, teach them how to use
a telephone in emergencies. They should
know how to call their home phone number
and the Operator. Use a toy phone for
practice, or hold down the hook on your
regular home telephone while the children
practice dialing.

servicio Si su telefono esta descompueslo por 24

Entre sus obligaciones esta la necesidad de ceder
una linea telef6nica en una emergencia. Ley Estatat
impone multas y periodos de carcel si Ud. no cede la
linea durante una emergencia o si se apodera de una
linea teletonica declarando falsamente que la linea
se necesita para una emergencia. La ley define una
emergencia como "una siluacion en la cual
propiedad o vida humana esta en peligro, y el pedir
ayuda inmediatamente es esencial."
Bajo Reglas Gubernamentales Federales Ud. puede
proporcionar su propio equipo telefonico y conectarlo
direclamente a la red telef6nica nacional Usted debe
de informar a Mountain Bell si lo hace, el equipo
debe de ser aprobado por la Federal

llamadas por mes. Por llamadas sobro esa cantidad
a Ud se le cobra 100. A Ud. se le cobra 200 por
llamadas hechas a traves del Operador(a) o sea "O."
Hay vanas formas de evitar cobros excesivos "DA'
(Asistencia del Directorio). Siempre vea su Directorio
pnmero. Si Ud. necesita dos numeros, pidale al
Operador(a) DA arnbos al mismo tiempo. Este
seguro del nombre de la persona con la que Ud.
desea comunicarse y como se escribe, siempre
escnba la informacion para que Ud. no tenga que

marcar llamadas de larga distancia directamente. sin
llamar al Operador(a). Marcando sus propias
llamadas de larga distancia, Ud. pagara los precios
mas bajos. y pagara solamente por los minutos que

Este es el tipo tie llamada de larga distancia mas
costosa. Llame tie persona a persona cuando Ud.

o numero de extension Marque la llamada y cuando
el Operador(a) enlre en la linea, diga que Ud. esta
haciendo una llamada de persona a persona y de el
nombre de la persona con quien desea hablar. El
Operador(a) saldra de la linea cuando la persona a la
que Ud. esta llamando haya contestado el telefono.
Llamadas con Tarjeta de Credito
Si Ud. tiene una Tarjeta tie Credito para Telefono Ud.
puede cargar las llamadas de larga distancia a su
cuenta de telefono. Marque la llamada y cuando el
Operador(a) entre en la linea. diga que Ud. esta
haciendo una llamada de Tarjeta de Credito y de el
numero tie su Tarjeta de Credito. Las Tarjelas de
Credito pueden ser obtenidas de su Oficina de
Negocios sin costo alguno.
Llamadas a Tercer Numero
Usted puede tener llamadas de larga distancia
cargadas a un tercer numero de telefono autorizado.
Marque la llamada y cuando el Operador(a) entre en
la linea diga que a Ud. le guslaria cargar su llamada
a un tercer numero y de al Operador(a) ese numero
