examination for accident insurance for any accident insurance company or association the minimum fee shall be five dollars ($5). For each examination for any local Fraternal association or lodge the minimum fee shall be two dollars ($2).
Resolved,
That the payment of all fees shall be authorized by the home office of the society or corporation for which such examination is made; and under no circumstances shall any examiner receive or accept any part of the fee from any agent or any other person or corporation unless the full fee is paid by authority of the home office and through its regular channels. Local fraternal associations and lodges where the rule of such association or lodge is for the applicant to pay for his own examination shall not be subject to these conditions.
