State Plan October 01, 2001 thru September 30, 2006
Education
All students with developmental disabilities, including students from culturally diverse communities and underserved and unserved areas in the state of Arizona, will have full access to a free and appropriate public education with additional academic supports as necessary to allow for equal participation within their community
7,500 parents and students statewide will be knowledgeable on their roles, rights and responsibilities under the Individuals with Disabilities Education Act (IDEA) Students with developmental disabilities in 1 urban and 1 rural area school district will have access to the related support services needed to succeed in the educational setting 600 media events, statewide, will portray individuals with developmental disabilities who are productive, independent and included in their communities, with at least 300 events demonstrating the positive impact of special education 4,500 parents and students with developmental disabilities will be informed on their roles, rights and responsibilities in the area of transition and will understand the vital role that students can play, throughout the educational process, in the development of their own Individual Education Plan (IEP)
Employment
Individuals with developmental disabilities, including individuals from culturally diverse communities and underserved and unserved areas, will be able to work without discrimination or losing needed benefits, while achieving and sustaining a quality of life that allows for choice, maximum independence and respect
Persons with developmental disabilities will be able to keep their health care benefits when they return to work with the passage of Medicaid buy-in legislation 500 individuals with developmental disabilities, employers, professionals and families reflecting the diversity of the state will know their rights and opportunities under the Ticket to Work and Work Incentives Improvement Act and the Americans with Disabilities Act.
Individuals with developmental disabilities, including individuals from culturally diverse communities and underserved and unserved areas, will decide who provides the services, and what services are appropriate and select employment goals that are meaningful to them
10 individuals with developmental disabilities who are currently in sheltered workshop settings will be employed in appropriate competitive employment with necessary supports 50 individuals with developmental disabilities, family members of persons with disabilities, employers and/or potential employers, will have been educated on natural employment supports and natural employment support plan development and implementation
Individuals with developmental disabilities, including individuals from culturally diverse communities and underserved and unserved areas in Arizona, will be afforded the maximum opportunities and funding for employment services
A study will be completed to identify employment services and support, conduct gap analysis, identify barriers and make recommendations including advocacy strategies for improved services and supports
Community Supports
All individuals with developmental disabilities and their families, including individuals from culturally diverse communities and underserved and unserved areas, have plans for successful aging
7,500 individuals with developmental disabilities and/or their families statewide will have knowledge regarding the full range of legal options, such as special needs trusts, power of attorney and guardianship
All people with developmental disabilities and their families, including individuals from culturally diverse communities and underserved and unserved areas, will receive information, advocacy and supports that they need throughout their lifetime for full community involvement
The Council will conduct 25 outreach activities aimed at providing information to individuals with developmental disabilities and their family members about the services and programs sponsored by the Council 6 existing adult recreation programs will include individuals with developmental disabilities in their programs
A minimum of 40 individuals with developmental disabilities in both rural and urban areas will have access to recreation programs in their communities 200 adults with developmental disabilities and their families statewide will be informed about aging issues and futures planning/retirement 4,000 individuals with developmental disabilities and their caregivers, statewide, will have information needed to plan for emergency situations 100 individuals with developmental disabilities will have access to a quarterly updated statewide accessible and affordable multi-family and single-family housing database 100 local government infrastructures, housing developers, architects and statewide housing agencies will be informed and educated about the affordable and accessible multi-family and single family housing needs of individuals with developmental disabilities Individuals with developmental disabilities in urban areas and 25 remote areas will be able to access a web site that provides local, state and national information on developmental disabilities 75 individuals with developmental disabilities will be supported in their advocacy efforts towards accessible transportation in their local communities 75 individuals with developmental disabilities will have access to and be informed of accessible transportation in their local communities 10 individuals with developmental disabilities under the age of 22, residing in a correctional facility and being served in a correctional facility will participate in a mentoring program
Self-Advocacy
All people with developmental disabilities, who qualify as individuals with developmental disabilities under the federal definition of developmental disabilities, across the state of Arizona including those from culturally diverse, unserved and underserved communities will have access to an issue based organization run by and for self-advocates
A statewide, self-sustaining coalition led by individuals with developmental disabilities will be established with a member base of at least 50 people who address issues relevant to their future and success
250 youth and adults with developmental disabilities statewide will have a greater awareness of their leadership potential and have been provided leadership opportunities The Council will support the active participation of 6 individuals with developmental disabilities in, one or more, cross disability and culturally diverse, statewide issue based, disability advocacy coalition 750 people, statewide, with developmental disabilities and their family members will be actively engaged in systems advocacy about quality assurance
Cross Cutting
Members of the general public and policymakers, statewide, will be educated about issues impacting the lives of individuals with developmental disabilities and be informed about the Council's initiatives
250 policymakers will be educated by the Council and the grassroots advocacy network about issues impacting the lives of individuals with developmental disabilities and be informed about the Council's initiatives 5,000 members of the general public will be educated about issues impacting the lives of individuals with developmental disabilities and be informed about the Council's initiatives
