PURPOSE
The purpose of this report is to provide
information concerning injury mortality among
Arizona residents. The data for 2000 are placed in
a temporal context by comparison with the data
for the preceding ten years.
This publication incorporates the contents of the
five previously published reports on suicide
mortality, firearm-related deaths, drug-related
mortality, unintentional drowning deaths, and
injury mortality among children and adolescents.
The specific health status information focused on
selected types of injury death or selected
population subgroups is supplemented with a
broad overview of patterns of injury mortality
both by the “intent” or manner of injury death
(that is accidents, homicides, suicides, etc.), and
“mechanism” of injury (for example, drowning,
firearms, drugs) among all residents of the State.
The new format of injury mortality statistics
reflects breaks in comparability of mortality data
and continuity in statistical trends effective with
deaths occurring in 2000. Beginning with the
2000 data year in Arizona (1999 nationally) two
major changes have occurred that affect the
computation of mortality rates, tabulation of
leading causes of death and analyses of mortality
data over time. First, a new revision of the
International Classification of Diseases (ICD),
used to classify causes of death, was
implemented. The Tenth Revision (ICD-10) has
replaced the Ninth Revision (ICD-9), which was in
effect since 1979. Second, a new population
standard for the age adjustment of mortality
rates has replaced the standard based on the
1940 population and used since 1943.
Any comparison of causes of mortality in Arizona
between 2000 and previous years needs to take
into account the changes in statistical trends that
can be attributed to changes in the classification
system and/or changes in the age-adjustment
method. Age-adjusted rates can only be
compared to other age-adjusted rates that use
the same population standard. The age-adjusted
mortality rates in this report, based on the (new)
2000 standard, CANNOT BE compared to rates,
which use the 1940 standard population. In
addition, the implementation of ICD-10 changed
the coding rules and rules for selecting the
underlying causes of death. Moreover, cause-of-death
titles have been changed and the cause-of-death
categories regrouped. In particular, the
new categories for neglect, abuse, and
maltreatment as causes of death, or categories of
specific types of drugs causing fatal poisoning are
not comparable to those used before 2000.
METHODS AND SOURCES
Data on the number and characteristics of injury
deaths among Arizona residents were obtained
from the death certificates filed with the Arizona
Department of Health Services.
The causes of injury death for 2000 are classified
by ICD-10. The causes of injury death for 1990-
1999 are classified by ICD-9.
Population denominators for Arizona residents,
used to re-calculate the age-adjusted mortality
rates, are the latest available estimates for 1991-
1999, released in December 1999 by
the Population Statistics Unit in the
Arizona Department of Economic Security
(www.hs.state.az.us/plan/popsets19.htm) and census
enumerations for 1990 and 2000 from the U.S.
Census Bureau (www.census.gov).
In order to increase the reliability of injury-specific
proportions and rates, many of them
were calculated for a full eleven-year period
1990-2000. Eleven year summary rates provide
more accurate descriptions when the number of
events is small and or the events occur in small
populations. The 1995 population counts (i.e.,
population at midpoint) multiplied by eleven,
were used as the denominators for average
annual rate calculations.
DATA ORGANIZATION
Injury Mortality Among Arizona Residents, 1990-
2000 is divided into 9 sections. Sections 1
through 6 are focused on the “intent” or manner
of injury death and provide an overview of all
injury deaths, accidents, intentional self-harm
(suicide), assault (homicide), legal intervention,
and events of undetermined intent.
Sections 7 through 9 are focused on three specific
“mechanisms” of injury mortality: firearms,
drugs, and drownings.
It is hoped that Injury Mortality Among Arizona
Residents, 1990-2000 will be a valuable resource
for public health professionals, policy makers and
all those who are interested in reducing the risk
of injury: the leading cause of preventable death
and disability in Arizona.

Injuries are the leading
cause of death among
Arizonans ages 1 to 5 and 9
to 33 (Figure 1-1). In the
eleven-year period from
1990 to 2000, unintentional
injuries in accidents,
combined with self-inflicted
injuries in suicide and
injuries purposely inflicted
by other person, accounted
for 9.0 percent of total
deaths (34,490 injury
deaths among 383,463
deaths from all causes in
1990-2000). Among
adolescents and young
adults ages 14-21, injuries
have claimed 4.3 times as
many lives as all other
causes of deaths combined
(4,199 vs. 968), accounting
for more than 81 percent of
total deaths of these
persons in 1990-2000
(Table 1-1).

In 2000, the death rates for
total injuries ranged from a
low of 22.2/100,000 in Santa
Cruz County to a high of
141.5/100,000 in Apache
County (Figure 1-5), a more
than six fold difference.
Compared to the state rate of
64.9/100,000, Apache
County’s rate was 118
percent greater, Navajo
County’s 46 percent greater
and Gila County’s 43 percent
greater. The three major
events accounting for the
injury fatalities, demise by
one’s own or another’s hand
or unintentional injury in an
accident, were the least likely
among the residents of Santa
Cruz, Yuma and Graham
Counties

excessive injury mortality risk
in Apache County, fatal
injuries accounted in 2000 for
almost 20 percent of the total
deaths to its residents
(Figure 1-6). The proportion
of injury deaths in Coconino
and Navajo Counties also was
excessive (16.8 and 13.4
percent, respectively). These
three counties accounted in
2000 for 52 percent of all
American Indian residents of
Arizona (133,068 out of
255,879), the group at the
highest risk of injury death.
While American Indians made
up only 5 percent of the
State’s total population, they
accounted for 77 percent of
the residents of Apache
County, and 48 percent of the
Navajo County population.

biological substances,
drownings, and exposure
to smoke, fire and flames
were the five leading
causes of unintentional
injury death in Arizona in
1990-2000 (Figure 2-2).
Excessive heat due to
weather conditions was
the sixth leading cause of
unintentional injury death,
followed by choking and
poisoning by gases and
vapors. Small aircraft
accidents causing fatal
injury to occupant were
the tenth leading cause of
accidental injury death in
Arizona.

In 1990-2000, the
average annual death
rate for motor vehicle
crashes among Arizona
males was 2.2 times
that for females
(Figure 2-3).
Poisoning by drugs
ranked second for
males, but third for
females. In contrast,
deaths from falls
ranked second for
females, but third for
males. The fourth and
fifth causes of
accidental injury death
– drowning and smoke,
fire and flames – were
the same for both
males and females.
Figures 2-4, 2-5, 2-6,
2-7 and 2-8 show
variation across
race/ethnic groups in
the five leading causes
of unintentional injury
mortality. The five
groups share most of
the leading causes in
common. For some of
the racial and Hispanic
origin groups, one of
the five leading causes
is exclusive to that
group. For Asians,
deaths due to
accidental discharge of
firearms ranked fourth,
but they were not
ranked in the top five
for the four other
race/ethnicity groups.

737 Arizona residents
committed suicide, a
decrease of 36 or 4.7
percent from the 1999
figure of 773. Between
1998 and 2000 the
Arizona suicide mortality
rate declined by 18.4
percent to 14.6 suicides
per 100,000 persons, the
lowest rate of the decade
(Figure 3-1).
In the 1990-2000 period,
no suicide death rate
among Arizona’s males
was below 24.0/100,000.
In contrast, none of the
annual female death rates
from suicide exceeded
8.8/100,000 during that
period

The suicide rate for young
adults 20-24 years old
declined by 60.6 percent
from 37.3/100,000 in
1994 to 14.7/100,000 in
1999, but it increased to
22.9/100,000 in 2000
(Figure 3-2).
The suicide rate among
adolescents 15-19 years
old dropped 55.3 percent
from 1997 to 1999, and
then slightly increased in
2000. The 2000 rate of
11.1/100,000 was the
second lowest suicide
death rate for Arizona
adolescents in two
decades.

In 1990-2000, all
annual age-specific
suicide mortality rates
among elderly
Arizonans aged 65 and
older exceeded by at
least 34 percent the
corresponding U.S.
rates (Figure 3-3).
However, the elderly
suicide rates were
lower in the years
ending (1996-2000)
than the years starting
the decade (1990-
1995).
Compared to 1990, the
Arizona elderly
residents 65 years and
older were 25 percent
less likely to kill
themselves in 2000.

those 45-54 years old
(10.0/100,000).
The 2000 suicide death
rate among males 20-
24 years old
(39.8/100,000) was
9.7 times greater than
the corresponding
female rate of
4.1/100,000.

rate among Black females
was the highest gender-specific
rate among
race/ethnic groups in
Arizona (Figure 3-7).
In 2000, no suicide deaths
were reported among
Black or Asian females in
the State. In contrast,
American Indian females
died from their own hand
at higher rates in 2000
than in 1990.
Married Arizonans clearly
were the least likely to
end their own lives in
2000 compared to
Arizonans in other marital
statuses (Figure 3-8).
Marital disruption in the
form of divorce or death
of a spouse was a factor
that increased the risk of
committing suicide. A
divorced person was 3.4
times, a widowed person
2.5 times, and a never
married person 1.9 times
more likely to end his/her
own life than a married

increases in homicide
mortality from 1990 to
1995 (Figure 4-1)). From
1995 to 2000, the homicide
mortality declined by 39.2
percent to a rate of
7.6/100,000. The change
from ICD-9 to ICD-10 did
not substantially affect
mortality patterns for
homicide.
The homicide death rate
among Arizona females
decreased from
4.7/100,000 in 1999 to
3.8/100,000 in 2000. The
homicide death rate among
Arizona males declined
from 13.6 in 1999 to 11.1
in 2000.

The 2000 homicide rates
were substantially higher
among Black, American
Indian and Hispanic
residents of the state
compared to homicide rates
among White non-Hispanics
and Asians. Blacks were 5.7
times more likely, while
American Indians 4.6 times
and Hispanics 3.7 times
more likely to die from
assault than White non-
Hispanics (Figure 4-2).

ASSAULT (HOMICIDE)
The 1995 homicide death
rate of 12.1/100,000
exceeded the 1990 rate by
49.4 percent. The number
of Arizonans who were
murdered increased from
302 in 1990 to 530 in 1995
(Table 4-1). The 1995
homicide rates were
particularly high among
adolescents 15-19 years old
and Arizonans aged 20-54
(Figure 4-3).
